ACL 14-25: Coordinated Care Initiative, Care Coordination Team (3/27/14)
In 2012, the Coordinated Care Initiative (CCI) was implemented. It is a model of care includes person-centered coordination of components, one of which is Interdisciplinary Care Teams (ICT). It combines home and community-based, primary care, and other Medicare and Medi-Cal services into one benefit package delivered through an organized delivery system administered by a participating Managed Care Health Plan. DHCS MEDIL I-14-21: Statement of Citizenship, Alienage and Immigration Status Form (MC 13) Under the Affordable Care Act
This letter, available here, clarifies when counties should send the MC 13 for immigration status. Applicants who claims to be citizens/nationals or have adequate immigration status on their applications will be run through the federal data hub in CalHEERS. If status can be verified this way, the MC 13 is not necessary, and the county can grant full-scope benefits. If status cannot be verified through the hub, applicants will be provided a 90-day reasonable opportunity period to resolve the matter before having benefits reduced.

ACL 14-28: Expansion Of The Definition Of Relative For The Federal KIN-GAP Program (3/25/14)
Instructions on the implementation of the expanded definition of “relative” for the federal KinGAP program. (No change to the state-funded KinGAP). In addition to the traditionally defined relatives, California can now provide federal KinGAP to “fictive” relatives. These are: non-related extended family members; adult members of the child’s tribe or an Indian custodian, and a current foster parent who has established a family-like relationship with the child and who enters into an agreement to be a permanent connection for the child. These new definitions cannot be retroactive to the period prior to federal approval, even though the state law exercising the state optional expansion passed in 2013.
